Removing Title from IE Title bar: 

1.Start -> run -> type regedit to open the registry editor.
2.Locate the following key : HKEY_CURRENT_USER\Software\Microsoft\Internet Explorer\Main 
3.Right Click on the value "Window Title" (on the right hand column of the registry editor) and delete it.
4.Click on yes to confirm deletion.
5.Close the registry editor and restart the computer. 

Removing the icon (top right): 

1.Close IE and explorer before going to the next step.
2.Start->run-> and then type in:
3.RunDLL32.EXE IEdkcs32.dll,Clear (case sensitive).
4.Restart computer and that's it.
